A competent person in this CU shall be able to conduct site inspection to collect evidence, carry out accident investigation, identify probable cause(s) of accident, prepare incident report and follow up on investigation results The outcome of this competency is systematic investigation of playground accidents to identify the cause(s) and propose remedial actions to prevent recurrence of the accidents. |
