| Penerangan CU | Community Based Rehabilitation Case Management describes the competency in assessing, planning, facilitating, care coordinating, evaluating, and advocating for options and services to meet an individual’s and family’s comprehensive health needs through communication and available resources to promote quality cost-effective outcomes. The person who is competent in community rehabilitation case management shall be able to assess client’s needs for rehabilitation and social support, develop rehabilitation care plan, monitor case management implementation and evaluate case management outcome. The outcome of this competency is to improved quality of life for PWDs and their families or caregivers and better community inclusion and participation whereby they reach their optimum level of wellness, self-care management, and functional capability. |
